Case Manager
We know that sometimes it can be challenging to attend your therapy sessions. Our case manager is here to help address your concerns. We can help with referrals to community resources, transportation services and financial counseling, or help you access funding assistance for items or services that aren’t covered by your insurance plan. We can also meet with you and your family to address barriers to your care; establish support systems; discuss your return to home, school or work; and hold conferences to talk about your goals, progress, and any questions or concerns about your rehabilitation.
Referrals
All therapy services require a physician referral.
Sports medicine/athletic trainer treatments require a physician referral. However, sports injury evaluations may be initiated by self-referral or by a coach, family member, physician or other allied health care professional.
