HRSA Grant Information
In 2013, Cox Medical Center Branson received a $500,000 grant from Health Resources and Services Administration for primary care quality improvement.
The grant provides for a population health management tool, the addition of a Care Manager and Behavioral Health consultant, and helps identify lean projects.
Using grant funding, Phytel system was purchased to assist with population health management. The tool is aimed to improve wellness and preventative measures through improved patient outreach, care coordination, patient engagement and to identify care gaps. The Phytel system is used in four primary care clinics – CoxHealth Family Medicine and Obstetrics, CoxHealth Family Medicine Branson Hwy. 248, CoxHealth Family Medicine Branson West and CoxHealth Family Medicine Kimberling City.
A Care Manager and Behavioral Health Consultant was added to help identify patients who could use assistance in self-management.
Through the lean projects, areas of waste and opportunities for improved efficiency have been identified.
Since implementing these programs:
- Diabetics with their blood sugar under control has gone from 40 percent to 70 percent
- Emergency Department visits by patients with chronic health conditions has decreased by approximately 24 percent.
- Diabetics with cholesterol under control has improved from 11 percent to 49 percent
- Tobacco intervention improved from 43 percent to 68 percent
- Depression screens have increased from 74 percent to 75 percent.
