Short-Term Disability is available to full-time and part-time employees (in a requisitioned position of 40 hours (.5 FTE) or more per pay period). This plan pays a weekly benefit directly to you in the event of a disability resulting from a covered accident, illness, or pregnancy that results in loss of income for greater than 7 calendar days.
Benefit Includes:
- Benefit: Premiums vary depending on age and salary. Benefit pays 60% of base salary following a 7-day elimination period.
- Enrollment:
- First day of the month following date of hire
- Within 30 days of a qualifying life event
- During Open Enrollment
- Effective:
- New Hires enrolling within the first 30 days will have guaranteed issue/acceptance for the policy and will be effective first of the month following 30 days of continuous employment
- After 30-day date of hire or application for coverage beyond the guaranteed issue amount, evidence of insurability (EOI) will be required, and the carrier will determine acceptance
- Late enrollment will be first of the month following approval of acceptance by the carrier

Long-Term Disability is available to full-time employees (1.0 FTE) on the first day of the month following one year of continuous employment. CoxHealth provides full-time employees who have been employed one year or longer with Long-Term Disability coverage at no cost to the employee. This policy provides income protections if you are unable to work due to a prolonged disability as a result of a covered accident or covered illness. Enrollment and coverage take effect automatically.

- Enrollment: automatic enrollment for eligible employees
- Effective: first day of the month following one year of continuous employment
