As your community partner, we're committed to helping your employees and their families maintain a continuum of care as they transition to the comprehensive CoxHealth network of providers and state-of-the-art facilities. We understand change can be difficult, but we believe it doesn't need to be!

We've built a Transition of Care Team to help you and your employees gain access to the care you need, when you need it. This team includes leaders from areas across the CoxHealth network, which ensures your company will be in good hands throughout these changes.

We make extra effort to provide you and your employees with everything you may need during this time. Maps of our facilities, lists of providers accepting new patients, and helpful contact lists will be available. We'll also provide a dedicated information line for your employees to call, and develop a webpage specific to your organization that's packed with helpful information and links.

We know that one size doesn't fit all, so we are happy to customize a plan that meets your needs. We want this to be a positive experience for everyone, and we'll work hard to make that happen!
